To assure our home care patients receive the most advanced treatments, we offer Specialty Programs to complete each individual’s care plan. These programs offer increased independence and improved quality of life for patients who receive care in the home. Our Programs are designed to measure and manage chronic diseases.
Our Specialty Programs deliver responsive, thoughtful, attentive care that utilize current technologies and therapies to patients’ homes. Our Specialty Programs are administered by experienced home health professionals who extend the training to patients and their families so therapies can be reinforced on a regular basis.

This program assists individuals who are recovering from cardiac related surgeries or illnesses or who are living with chronic cardiac disease. We work with you, your caregivers, and your care providers to ensure that you receive the appropriate monitoring and education. Home Health Specialty Services, Inc. offers experienced cardiovascular nurses, physical, occupational, and speech therapists, social work services, and home health aides to assist you in your recovery. Home Health Specialty Services staff will work with you and your caregiver to increase your knowledge of cardiac disease and recovery; ensure you are able to comply with your medication regimen and rehabilitation program; help you and your family to cope with disease or illness; assist hospitals and physicians in decreasing your length of stay; and decrease the incidence of re-hospitalization and your need for emergency care.
This program assists individuals living with chronic obstructive pulmonary disease ease symptoms, decrease the risk of complications, and improve quality of life. The staff at Home Health Specialty Services is experienced at working with individuals with COPD and their family members. Our home health team uses a multidisciplinary approach of nursing care and therapy. Nurses educate individuals and families on the common triggers of COPD and how to manage them and reduce hospitalizations. Education is provided on breathing techniques, diet, medication compliance, fluid intake, and oxygen therapy. Physical therapy is imperative and may help individuals through the implementation of an exercise regimen designed to strengthen the pulmonary system.
Home Health Specialty Services offers therapy and treatment following joint or orthopedic surgery. There are four performance measures specific to after care from joint and orthopedic surgery which include the assessment of pain, difficulty breathing, activity of daily living, and range of motion. Physical and occupational therapies are offered to enhance rehabilitation. Home health aides are available to assistant with personal care. Social services are available to aid individuals and families in resolving any emotional issues or to assist with coping. Home Health Specialty Services works with individuals to coordinate the delivery of necessary durable medical equipment to the individual’s home. Skilled nurses remove sutures/staples, manage surgical dressings, monitor pain management, perform safety assessments, and monitor PT/INR levels in the home.
The nurses at Home Health Specialty Services have significant experience with treating individuals with diabetes. Home Health Specialty Services, Inc.’s professional staff work with individuals and families to reduce preventable hospitalizations and medication adverse effects. Home Health Specialty Services will assist individuals and their families to develop an appropriate treatment plan including self-blood glucose monitoring, preparing/administering injectable insulin, and educating on signs, symptoms of, and treatment for hypoglycemia/ hyperglycemia. Home Health Specialty Services staff follow the national standards of care to prevent complications. Nutrition and exercise is important in the management of diabetes and the staff at Home Health Specialty Services, Inc. incorporate these components into the care provided as well.
The Wound Care Program at Home Health Specialty Services, Inc. is a specialty program that focuses the provision of individualized and expert care under a patient's physician order to ensure our patients reach optimal healing and positive outcomes. Wound healing is a complex so this program offers a multidisciplinary approach that addresses the treatment of the wound and the underlying disease process. Evidence-based treatment protocols based on standards of care developed by the Wound, Ostomy, and Continence Nurses Society and the National Alliance of Wound Care are implemented with continuous assessment and reassessment of each wound. Our staff is highly trained and utilize cost-effective and clinically proven products to make certain our patients are receiving superior care.
The staff at Home Health Specialty Services, Inc. Has extensive knowledge on how to care for individuals who have a stroke. The staff focus on recover and prevention of secondary symptoms associated with inactivity such as stiff joints, falls, bedsores and pneumonia. This is accomplished by the restoration of abilities to perform independent activities as soon as possible after an individual’s return home. Our nurses offer plans of care that work toward prevention of future cerebral vascular stroke methods, safety measures, medication management, proper diet, and monitoring of vital signs as well as head to toe physical assessments. Physical therapy, occupational therapy, and speech therapy are available to assist with recovery as well as social workers who may treat some of the emotional consequences of an individual’s health status. Home health aides are available to assist with activities of daily living until recovery occurs.
Behavioral Health Care
Home Health Specialty Services, Inc. plays a role in the treatment of individuals recovering from mental illness. Home Health Specialty Services has qualified psychiatric nurses and social workers who are experienced in creating individualized home care plans that include medication safety and compliance, education on coping skills, reducing hospitalization, assisting with accessing services, and assistance with activities of daily living. Other services provided include medication administration and medication pick-up and delivery. Nurses are available to make home visits seven days per week and our nurses are skilled at crisis intervention.
